Terms And Conditions
All new Patients are required to read and confirm the acceptance of the terms and conditions of this therapeutic agreement.
Confirmation can be provided in writing via email. If written confirmation is not provided for any reason, then by commencing therapy the Patient agrees to the term & conditions of this agreement. The Patient is required to respond in writing (via email) if s/he does not agree to the terms and conditions of this agreement.
The Psychologist abides by the code of confidentiality of IAHP, IACP, RCI and AIP regulations. Identifying information, such as Patient’s name, address, biographical details and other description of a Patient’s life and his/her circumstances, will be kept confidential and stored safely.
Definations :-
1.The ‘Psychologist’: The clinician assigned by Hearts Happiness Psychology Clinic Jodhpur Ltd to offer psychological services.
2.The ‘Patient’: You
3.Therapy’, ‘Therapy session’ ‘Appointment’ and/or ‘Therapy appointment’ (in-person and/or online/remote) is the time spent with the Psychologist who will apply his chosen method of therapy application and modality.
4.Modality’ is a model of study for therapeutic application. Psychodynamic Therapy, Cognitive Behavioural Therapy, Eye Movement Desensitisation and Reprocessing are examples of psychological modalities.
5.The ‘Session’ and/or ‘Appointment’ (in-person and/or online/remote) is the booked time slot and arranged time working with the ‘Psychologist’.
6.‘Presenting Issue(s) or ‘Problem’ is what the Patient wants to work on with the Psychologist.
7.The ‘Goal’ is the therapeutic goal created collaboratively with input from both the Patient and the Psychologist and is what the patient would like to achieve as the desired outcome in therapy.
8. The ‘Desired Outcome’ is the desired result of the work that the Psychologist and the Patient work towards achieving.
9. The ‘Session Time’ in-person and/or online/remote is 50-or 75-minutes unless otherwise agreed.
Formation Of Agreement :-
1. Counseling is a confidential process based on the concepts of psychology and designed to help clients address their concerns, come to a greater understanding of themselves, and learn effective personal & interpersonal coping strategies. It involves the disclosure of relevant ‘personal information’. Thus, for counseling to be effective, complete honesty is solicited on the client’s part, as the counselor primarily works with the information and goals shared by the client.
2. During the course of counseling, clients often learn things about themselves that they don’t like, or revisit and confront aspects of their life that they usually avoid. This may cause brief periods of increased distress or confusion. Though the counselor will caringly handhold the client through such temporary phases, at any point of time, if the client finds the distress unbearable then the client is supposed to share this immediately with the counselor. The ensuing progression can then be mutually decided.
3. Discussions between the counselor and client are completely confidential. No information will be released without the client’s written consent unless mandated by law. Moreover, even if the nature of the problem is ever discussed in academic or research contexts, it will be duly reformulated to preserve confidentiality. The client’s identity or identity-markers will never be revealed in any case. As a variant of confidentiality, it is the client’s right not to disclose to their kith and kin about availing of the counseling services. However, the counselor remains rightfully answerable to any affected party in such a case.
4. The success of counseling depends upon the quality of efforts of both the counselor and the client. So, while the counselor shall remain totally committed to helping the client reach the counseling goals, it has to be acknowledged that the client is primarily responsible for the application of choices/changes discussed in counseling and thus for the outcome of the counseling process. Moreover, the counselor cannot be held responsible for the misinterpretation of any of the contextual discussion-threads by the client.
5. The number of ‘counseling sessions required’ depends primarily on counseling goals. While the client can mention their preferred number of sessions to the counselor right at the outset and the counselor shall customize the process accordingly, both the counselor and the client are free to propose an extension to the number of sessions, and intervention can be extended with mutual consent and revision of goals. However, the client reserves the right to stop availing of the counseling services at any point in time. Similarly, the counselor reserves the right to stop offering services in case of any misconduct on the client’s part.
6. The client’s relationship with the counselor is a professional relationship. To preserve its sanctity, it is imperative that the counselor does not have any other type of relationship with the client. The counselor cares about helping you but is not in a position to be your friend or to have a personal association with you.
7. We do not counsel for psychiatric disorders, mental illnesses, substance abuse, rehabilitation, disability, or issues requiring clinical psychology interventions. We don’t counsel anyone with a disposition to harm themselves or others. We also don’t counsel minors without the presence or permission of a guardian.
8. The standard duration of our counseling session is 1 hour. We only counsel by appointment. We do not offer emergency services. The fee has to be paid by the client prior to the session. The fee is non-refundable.
By signing and reading this form, you acknowledge that you have read & understood all the above points, you voluntarily agree to receive counseling service, and you authorize the counselor to provide it as considered necessary and advisable.
